Our Mission
Our organization is a non-profit entity established with the mission to promote the physical and mental well-being of Hong Kong residents—especially the elderly, individuals with chronic illnesses, grassroots communities, and other vulnerable groups—and to advance health equity. Through community-based preventive healthcare services, we are committed to enhancing public awareness of disease prevention, care, and self-management, thereby benefiting the wider Hong Kong society.
To fulfill our charitable objectives, we carry out the following initiatives:
(1) We engage directly with communities to deliver health-related educational activities to grassroots residents and vulnerable populations. These include health seminars, self-assessment surveys and screenings, public health knowledge promotion, and chronic disease self-management training. These efforts aim to raise citizens’ understanding of disease risk factors, healthy living habits, and early warning signs;
(2) We provide individualized follow-up services for patients with chronic illnesses—particularly those from lower-income backgrounds. These services include patient-focused seminars, case referrals, psychosocial support, medication guidance, and resource matching with community services.
Our core aim is to ensure that patients, vulnerable groups, and individuals at risk of illness can equitably access accurate and timely health information—both preventive and disease-related. By doing so, we help reduce the impact of financial, geographic, and informational barriers on health management and actively promote health equity across the population.
